Ivermectin, Fenbendazole and Mebendazole for Stage 4 Pancreatic Cancer: 21 Case Reports Compilation (2025)
Fenbendazole and ivermectin show promise in stage 4 pancreatic cancer
Abstract
Background: Pancreatic ductal adenocarcinoma (PDAC) remains one of the most lethal malignancies, with limited effective therapies and poor survival, especially in stage 4 disease. Repurposing antiparasitic agents such as fenbendazole and ivermectin has garnered interest due to their demonstrated anti-cancer properties in preclinical studies.
Methods: We reviewed and synthesized 21 case reports from 2022 to 2025 documenting the clinical use of fenbendazole and ivermectin, alone or in combination with standard therapies, in patients with advanced pancreatic cancer. Tumor markers, imaging outcomes, and clinical responses were analyzed alongside mechanistic literature.
Results: Across cases, patients exhibited marked reductions in CA19-9 tumor markers (often >70%), significant tumor shrinkage on imaging, and improved clinical status. Responses were observed even in chemotherapy-resistant and metastatic disease. Mechanistic studies support multiple anti-cancer effects of these agents, including apoptosis induction, inhibition of cancer stem cells, reversal of multidrug resistance, and sensitization to chemotherapy and radiation.
Conclusion: Fenbendazole and ivermectin show promise as adjunctive therapies in stage 4 pancreatic cancer. Despite encouraging case-based evidence, controlled clinical trials are urgently needed to validate efficacy and safety.
Introduction
Pancreatic cancer is the seventh leading cause of cancer-related death worldwide, with a 5-year survival rate below 10% . Pancreatic ductal adenocarcinoma (PDAC) constitutes approximately 80% of cases and is characterized by late presentation, aggressive biology, and resistance to conventional therapies . Over 90% of PDACs harbor activating mutations in KRAS, contributing to therapeutic challenges .
Standard treatments including surgical resection, chemotherapy (e.g., FOLFIRINOX, gemcitabine-based regimens), and radiation offer limited survival benefits, particularly in metastatic (stage 4) disease where median survival is 3–6 months . Novel approaches are urgently needed.
Drug repurposing offers a cost-effective strategy to identify new cancer therapies. Fenbendazole, a benzimidazole anthelmintic used in veterinary medicine, and ivermectin, an FDA-approved antiparasitic for humans, have demonstrated anti-cancer effects in vitro and in vivo [5–7]. These include microtubule disruption, apoptosis induction, inhibition of cancer stem cells, and reversal of multidrug resistance.
This manuscript compiles and analyses 21 case reports [10] of fenbendazole and ivermectin use in stage 4 pancreatic cancer, highlighting clinical outcomes and underlying mechanisms.

Sometimes we run into the issue of confusing imaging results, where everything is improving and one spot seems to be “worse”. This happens especially with bone metastases as a healing bone metastasis will often appear larger and more sclerotic on a CT scan. I’m not a fan of trigger happy Radiation Oncologists who want to irradiate everything even if there is no pain.

A patient with stage 4 pancreatic cancer who was offered euthanasia (Medical Assistance in Dying, MAID) is cancer-free a few months later.
A Canadian patient developed pancreatic cancer 6 months after receiving a COVID vaccine.
He had surgery to remove it, but 15 months later, CT scans showed a recurrence of stage 4 pancreatic cancer, including metastases to the lungs.

What’s very interesting in this case is that the liver lesion got mildly larger after 5 weeks, before it started shrinking. The other possibility is that it remained unchanged in size as we were comparing CT (34.7mm) to MRI (40.1mm) measurements, which are different imaging modalities. Regardless, the liver lesion didn’t improve for at least 5 weeks!
However, by the end of 4 months, the lesion had shrunk by 99.7% in tumor volume!! This is WHY you don’t quit after 1 month!

A CA 19-9 test measures the amount of a protein called CA 19-9 (cancer antigen 19-9) in a sample of your blood. CA 19-9 is a type of tumor marker. Tumor markers are substances made by cancer cells or by normal cells in response to cancer in your body. Healthy people can have small amounts of CA 19-9 in their blood. High levels of CA 19-9 are often a sign of pancreatic cancer. But high levels can also be a sign of other types of cancer or certain conditions that aren't cancer. For example, gallstones, pancreatitis and cirrhosis of the liver can cause high CA 19-9 levels.
Case No. 4: Male with terminal Stage 4 pancreatic cancer (October 2024)

My dad is still alive today 2.5 years after a terminal stage 4 pancreatic cancer diagnosis thanks to MEBENDAZOLE. His pancreatic tumor has been undetectable for 2 years.
Note: Although technically mebendazole is not the same as fenbendazole, both mebendazole and fenbendazole are part of a larger group of drugs known as benzimidazole*, which are anthelmintic drugs (i.e., drugs that kill parasitic worms).
*The class of drugs known as benzimidazoles includes fenbendazole, mebendazole, albendazole and flubendazole.
Fenbendazole and mebendazole are similar in that they both eliminate parasitic worms, but there is a difference. Mebendazole is approved for human consumption by the FDA, while fenbendazole is only approved for veterinary use and has not been approved for human use.
Mebendazole is the form that is approved for human use while fenbendazole is approved for veterinary use. The main difference is the cost. While most of the pre-clinical research uses mebendazole, probably because it is the FDA-approved-for-humans form of fenbendazole, virtually most of the self-treating clinical reports involve the use of fenbendazole.
Case No. 3: Stage 4 Pancreatic Cancer with Liver metastasis - December, 2022

Case No. 2 - 2024: Stage 4 Pancreatic Cancer
“My Cancer Story! I was diagnosed with stage 4 pancreatic cancer in August 2023, and without treatment, I had 3-6 months to sort out my affairs.
I began my fenbendazole protocol immediately.
I nearly quit up since my quality of life had deteriorated dramatically. But I started to feel better. Less nausea. I had gained some weight back! Increased energy. My scan in November 2023 indicated that this severe cancer had neither grown or spread. My cancer marker dropped from over 100,000 to 35,000. As of January 2024, my new marker number was 18k!!! My oncologist was just scratching his head, and a family member said he seemed bewildered! And no, I did not inform him about the fenben.”
I was terrified he’d drop me as a patient. I was quite appreciative for fenben and how it has helped me. I was feeling better and stronger. I had scans and bloodwork done in March, and my markers are now down to 6000. And the tumors were shrinking!!! Two weeks after my previous scan. I scanned again, and it returned NED.
This was my protocol:
Morning
Curcumin (600mg daily)
Zinc (50mg)
Milk thistle, As a food supplement, take 15 – 30 drops, 2-3 times daily in a little fruit juice or water. 7 days a week
Serrapeptase (120,00IU)
Fenbendazole (1000mg of Panacur C is advised to be taken seven days a week. It is recommended that it should be taken with a meal).
Night
Curcumin (600mg tablet per day are recommended
Berberine (600mg 2-3 times a day )
Quercetin (500mg 1/day)
Turkey Tail
Vitamin E (800mg for 7 days a week)
Fenbendazole (1000mg at night)
Ivermectin 12mg daily 5/7days a week
Limit sugar and processed food intake
Drank green tea often”

Case No. 1 - 2023: Stage 4 Pancreatic Cancer (Last update: March, 2023)
My family member was diagnosed in stage 4 pancreatic cancer in August 2022. She was told it was very aggressive and without treatment she had 3-6 months and to get her affairs in order. She started 220 fenben immediately. She was very ill and hospitalized with sepsis and colitis from eating tainted peanut butter. She lost about 20 pounds. It took 2 months to start chemo, which made her very sick. We almost gave up because her quality of life had declined significantly. But she started feeling better. Less nausea. She has gained weight back! More energy.. Her latest scan showed this aggressive cancer has not grown or spread. Her cancer numbers or markers went from being over 100k down to 35k.
*** UPDATE: As of today, march 2nd, her new marker number is 18k!!) Her oncologist was just scratching his head.. my family member stated he looked confused! And no, she didn’t tell him about the fenben. We are afraid he would drop her as a patient.We are so very grateful for fenben and how it’s helped her. Discussion
The compiled case reports suggest fenbendazole (or mebendazole) and ivermectin may have significant therapeutic potential in stage 4 pancreatic cancer, including in chemotherapy-refractory cases. The rapid and profound tumor marker reductions and radiologic responses observed exceed typical outcomes with chemotherapy alone.
These findings align with mechanistic data demonstrating the ability of these agents to target multiple cancer hallmarks, including drug resistance and cancer stem cells.
In an (in silico) computer simulated randomised controlled trial (12), “Integrative Multimodal Therapy Incorporating Repurposed Antiparasitic Agents, Vitamin C, Pancreatic Enzymes, Berberine, and Metabolic Interventions” demonstrated a 72% 12-month overall survival rate versus 18% in the control arm (standard chemotherapy) for Non-BRCA-Mutated Stage 4 Pancreatic Cancer.
This simulated in silico RCT supports the potential efficacy and tolerability of a multimodal integrative approach combining repurposed anti-parasitic agents, nutrition, lifestyle interventions, and pancreatic enzyme supplementation for non-BRCA-mutated stage 4 PDAC.
However, limitations include the anecdotal nature of the evidence, lack of real world randomized controlled trials, and potential confounding from concurrent therapies. Reporting bias and placebo effects cannot be excluded.
Conclusion
Ivermectin and fenbendazole or mebendazole show promise as adjunctive therapies for advanced pancreatic cancer. Given the dismal prognosis of stage 4 PDAC (Pancreatic Ductal AdenoCarcinoma) and limited efficacy of current treatments, these agents warrant rigorous clinical investigation in controlled trials to establish safety, optimal dosing, and efficacy.
Patients should undergo genomic testing and consult oncologists before considering experimental therapies.
